New Patients

The Kingston Family Clinic welcomes new patients from our local area.

Making Your First Appointment
When calling the clinic for your first appointment please identify yourself as a new patient and if you have a particular doctor that you would like to see. (Please note that not all of our doctors are able to take on new patients at this present time. This is to ensure that they are able to offer the best possible care to their existing patients.) When making your appointment you will be asked to supply your personal details (address, DOB, phone number), provide your medicare card and any concession cards.

Attending Your First Appointment
Please arrive 5 mins early for your first appointment. As a new patient you will be asked to read & sign our clinic’s privacy & consent form. You will also be provided with a new patient medical history form to take in to your first consult. This form will give your new doctor an important “snapshot” of your health and medical history and will enable them to provide the best possible care for you and your family.

Transfer of Medical Records
You may also wish to arrange a summary of your clinical records to be transferred to our clinic. Our reception staff can assist your with this process, however your previous clinic may charge you for this transfer.
